Florida Department of Children and Families report on Nubia Doctor
December 21, 2004
Spoke with the Nurse Practitioner XXXXXXXXXXX(last name unknown), due to Nubia docter’s appointment that is scheduled for 12/21/2004. She wanted to make sure that Nubia did not miss this appointment because she has not had a follow up since January of 04. She stated that the child had to be seen by the docter 3x‘s a year. This counselor explained to the nurse that the foster parent never goes to the appointment with the children. She sends them with transportation. This counselor also told the nurse that when she calls the foster parent, she is going to tell her that she cannot take the child. The nurse explained to this counselor that the child is not in a good placement because the foster parent does not care for the child's well being if she does not attend the medical appointments with the child. The nurse also stated that the child may need to be in a medical foster care home. The nurse stated that she was going to call the head of the foster medical care units because this child is very medically needy and should not be missing appointments becasue the foster parent does not want to take her. She went on to state that maybe the brother could be placed with the child as well. She stated that she has a concem for the child if the placement is adoptive because the foster parent would then have the sole responsibility to care for the child and she does not believe that will happen. She has also stated that the children have not had their four year old shots. The children are in daycare without their shots. She does not know how this is possible.

Children seen for psychosexual evaluation. The children were appropriately dressed for visit. The children appeared to be happy and healthy. The daycare was neat and clean.
There were no signs of any hazardous conditions visible. The children were seen by Dr. Czchowicz, The doctor stated that the child,ubia stated that the foster parent, Mr. Barahona tickles her in front of eveyone. The Dr. stated that he does not think there is a I problem with the child. He went on to state that he had to report what the child said because he is required to do so. He also gave the children a date for the observational visitation, 2/7/05 @ 1pm contingent upon the father‘s schedule.

IN THE PAST, THE FOSTER FATHER (UNKNOWN) TICKLED NUBIA‘S PRIVATE AREA (VAGINA) WITH HIS FINGERS. THIS HAPPENED MORE THAN ONCE, AND THE INCIDENTS REPORTEDLY OCCURRED IN THE PRESENCE OF OTI-[ER ADULTS IN THE HOME. IT IS UNKNOWN WHEN THE INCIDENTS OCCURRED, AND IT IS UNKNOWN IF ANYTHING SUCH THING HAS BEEN DONE TO XXXXXX. THERE ARE OTHER CHILDREN in THE HOME, BUT IT IS UNKNOWN IF THOSE CHILDREN ARE ALSO FOSTER CHILDREN OR BIOLOGICAL CHILDREN OF THE PARENTS.

EDWARD SCZECHOWICZ IS A PSYCHOLOGIST THAT SAW NUB IA ON 1/26/2005. I·E STATED NUBIA WAS BROUGHT IN TO HIS OFFICE BY HER DCF WORKER BECAUSE SHE WAS SEE MASTURBATING RECENTLY, I·E STATED TO I-HS KNOWLEDGE APROX 40 PERCENT OF GIRLS AT NUBIA'S AGE MASTURBATE. HE WAS NOT VERY CONCERNED BECAUSE OF THIS. NUBIA DISCLOSED TO HIM THAT TI·E FOSTER FATHER TICLED HER PRIVATE PARTS. HE STATED TO CPI THAT HE FOUND HER STORY VERY QUESTIONABLE AND BELIEVES UNFOUNDED BECAUSE OF HOW SHE DISCLOSED TIE STORY AND BECAUSE OF CIRCUMSTANCES AROUND THE NARRATION OF SAID STORY. HE ALSO STATED SHE TOLD HIM OTHER ADULT MEMBERS WERE PRESENT. HE STATED IN HIS POSITION HE BROUGHT THIS ISSUE UP, HOWEVER DOESNT FIND MUCH VALIDITY TO NUBIA'S STORY. HE STATED TO CPI THAT IN HIS OPINION IT WOULD BE DETRIMENTAL TO WAKE THE CHILDREN UP AND CONFRONT THEM AT THIS TIME (AFTER 12AM).

F.F ALL FAMILY MEMBERS. BOTH MR AND MRS BARAHONA , THEY STATED THEY MADE CLEAR STATEMENTS THAT DENIED ABUSE ON THEIR PART, THEY STATED THEY HAVE MENTIONED IN MANY OCCASIONS TO THE CASEOWRKER MS HARRIS AND THE GAL, PAUL NEWMAN, ABOUT CONCERNS OF CHILDREN ACTING OUT SINCE VISITATION WITH BIOLOGICAL FATHER RESUMED. THEY ARE SURPRISED AS TO THESE ALLEGATIONS BUT ARE NOT SURPRISED AS TO CHILDREN'S PROBLEMS.
F/F NUBIA AND XXXX CHILDREN WERE INTERVIEWED INITIALLY SPEPERATELY THEN TOGETHER. NUBIA DENIED FO FATHER TOUCHED HER. BOTH CHILDREN DID MAKE STATEMENTS AS TO THEIR BIOLOGICAL FATHER. THEY APPEARED TO CALL BOTH DADDY WHEN SPEAKING IN ENGLISH BUT CALLED PAPA AND PAPI WHEN ADDRESSING THEM IN SPANISH CLEARLY DIFERENTIATING THEM. BOTH CHILDREN WERE SEEN WELL GROMMED AND DRESSED, FREE OF BRUISES/MARKS.

Children seen at home. The home was neat and clean and there were no signs of any hazardous conditions visible. The foster parents stated that they have bonded well with the children and want to adopt if the children do not return to their father. The foster parents talked about the allegations that were made and he stated that he would never molest any child. He stated that the Police and the P.I. came to the home and completed the investigation. he wanted to know what were the findings and if the case was still open. This counselor stated to him that the case was closed with no findings. He also was upset because the record would remain on file indefinately. The foster parents appeared loving and caring. They also have another child in the home named XXXXXX He is XXXXXXXXXXXXXXXXXXXXXXXX. They stated that they have room for one more child and the age range that is ideal is 0-5 years old. This counselor also explained that Nubia has a trust fund that is reaching capacity and for the foster parents to take the child to Walmart and place items on layaway that she needs so that we can make sure that the trust fund does not affect her SSI check. The foster parents stated that she needed some clothing and she would put the clothes on layaway as soon as possible. There are no services needed at this time. The children had visited the on 2/14/05 Docter and does not need any immunizations until they reach the age of 5. The foster parent stated that she took Nubia for a follow up from her surgery and the Docter stated that he doesn't need to see her until she is older. he stated that she did not need to bring her because the surgery was in 2000.

Case staffed with atty. Counselor did not have JRSSR for staffing due to the court case that was on in the morning calendar was pushed back into the afternoon calendar. Status of case. Father is not in compliance with services. Counselor spoke with and requested the compliance status of the father and agency stated that the father was in compliance She stated that the father was continuing sessions with the Docter Chechowiz. This counselor assumed that the agency as all other agencies would have the referral in the file and refer to it once this counselor calls about the compliance. This counselor stated to the atty that the referral was made for the father's services in 6/04. The father was in non compliance in the first 1"RSSR. This counselor handed the father the referral for his services. This counselor stated to the atty that although there may have been confusion on this counselor`s part with the agency, it is ultimately the parent's responsibility once they have the referral and sign the case plan to complete the services. The atty stated that the time was waisted on the case and this counselor stated to her that it was not waisted on my part. This counselor called the father and he stated that at first that he was attending parenting classes and then he stated that he was not. He stated that his atty told him that once he finished his services with Dr. Chechowicz that he would not have to do any other
services. There are no services for the mother, she has been TPR'd. This counselor is to take down the corrected JR to the atty for review on 2/18/2005.

Children seen at home. The home was neat and clean. There were no signs of any hazardous conditions visible. The children were appropriately dressed for the visitation. The children were playing with other children residing in the home and they were watching television. The foster parent stated the concems for the case pending against the father. She asked if there were any new details that this cotmselor could provide her, This counselor stated that I have not spoken with the Detective from the Police Department. When calls are placed, the answering machine picks up. The Detective has not called either. She stated that the father has court for the allegations that brought the children into care on 4/18/05. She also stated that the Officer stated to her that the father was not to have any contact with any children at all. The Officer stated that DCF does not know what we are doing. This counselor explained to the foster parent that the Court ordered a psycho-sexual on the father and the Psychologist recommedations were that of the father‘s risk to the children was low. The courts then deemed that he should have visitation with his children. The Department does not have a copy of the no contact order that is in effect in criminal court. This counselor and the foster parent is in agreement to keep in touch on any further developments that either may have information on. This counselor also explained to the foster parent that there are relatives in Texas that want to adopt the children. She stated that she understood that if there were relatives that the children would be placed with family first. She stated that if the relatives could not adopt then she and Mr. Barahona would be willing to sdopt. The children have no known illnesses at this time. There are no services needed at this time. Nubia‘s SSI was cancelled g and this cotmselor provided the foster parent with an emergency copy of the child's medicaid.

ON 02/23/06, NUBIA, THE CHILD, WAS NOT ICED WITH A HUGE BRUISE LOCATED ON HER CHIN AND NECK AREA. THE INJURY IS ABOUT THE SIZE OF A TAN GERINE. IT IS UNKNOWN WHAT HAPPENED TO NUBIA AND THERE ARE TWO VERSIONS; ONE IS THAT SHE FELL AT HOME, THE OTHER IS THAT SHE FELL AT SCHOOL. THERE ARE ALSO (SPOTS) BRUISE LOCATED ON HER BACK AREA. THERE ARE ALSO SCRATCI-IES LOCATED IN THE SAME AREA. NUB IA HAS ALSO MISSED SEVERAL DAYS FROM SCHOOL. THERE ARE OTHER CHILDREN IN THE HOME, NO KNOWN HARM TO THEM. RESPONSE PRIORITY: IMMEDIATE

PI WENT XXXX ELEMENTARY AND HAD A FACE TO FACE WITH THE VC. SHE WAS SEEN WITH A RED BRUISE ON THE LEFT SIDE OF HER CHIN. SHE ALSO HAD A RASH IN HER SCALP. SHE WAS NOT TOO CLEAN AT SCHOOL. VC WAS VERY AGILE AND TALKED A LOT. SHE WAS SEEN VERY HYPPER AND COULD NOT STAY STILL TO BE INTERVIEWED. SHE JUMPED AND WALKED ALL AROUND THE ASSISTANT PRINCIPAL OFFICE AND TOUCHED EVERYTHING SHE SAW INSIDE. SHE TOLD PI THAT SHE SLIPPED AND FELL IN CLASS THAT WAS WHY SHE GOT THE MARK ON HER CHIN. VC ALSO HAD A RED SPOT IN HER BACK AND DID NOT GIVE PI AN EXPLANATION ABOUT IT. SHE STD SHE DID NOT SAY IT TO HER TEACHER. SHE ALSO STD NOBODY HIT HER. WHILE TALKING TO IER, THE CHILD JUMPED IN THE MIDDLE AND SAID SHE SLIPPED AND FELL IN CLASS. THE FOSTER MOTHER STD VC HAS BEEN WITH HER FOR ABOUT TWO YEARS. SHE LOVES HER AND IS ABOUT THE ADOPT HER. SHE STD SHE TOOK GOOD CARE OF IER LIKE HER OWN CHILD. SHE STD THE CHILD IS GOING TO SEE HER DOCTOR SOON FOR THE REGULAR CHECK UP AND SHE WILL TALK TO TIE DOCTOR ABOUT THE SCALP PROBLEM. PI TOLD HER IT IS SOMETIIING MORE SERIOUS THAN THAT AND SHE SHOUL TAKE CARE OF IT ASAP. PI ALSO TOLD HER TO CALL CPT BY 10 AM AND CPT MAY ALSO TELL HER WHEN TO COME TO THE CLINIC WITH THE CHILD. FOSTER MOTHER'S PH VC PEDIATRICIAN DR. XXXXXXXXXXXXx. VC IS TAKING XXXXXX CHECKED THE HOME AND MADE FACE TO FACE WITH THE OTHER CHILDREN IN THE H0ME_2-1-03; GEORGE BARAHONA THE FOSTER HOME BIOLOGICAL CHILD 4-29-99. ALL THE CHILDREN WERE SEEN FREE OF MARK OF BRUISES. XXXXXX CALL NUBIA HIS SISTER AND SAID NOBODY HIT NUBIA. HE SAID NUBIA IS HIS GOOD SISTER XXXXXXXXX DID NOT SAY ANYTHING, THEY WERE IN TIE ROOM. TIEY WERE NOT TOO VERBAL. THEREFORE, THEY WERE NOT INTERVIEWED. ///////// THE CHILDREN ROOM WAS CLEAN AS WELL AS TIE REST OF TIE HOUSE. TIE HOUSE IS HUGE AND TIE FOSTER MOTHER OPENED TIE FRIDGE FOR PI AND THERE WAS PLENTY OF FOOD AND MEAT INSIDE. TIE UTILITES WERE ON TOO. THERE WAS A SIGN OF MAD DOG AT TIE GATE BUT NO PET WAS SEEN IN TIE YARD OR THE HOUSE EXCEPT FOR A CAGED BIRD.

PI WENT TO THE HOME AND MADE FACE TO FACE WITH THE FOSTER PARENTS. FIRST PI MADE FACE TO FACE WITH THE HUSBAND. HE STD HE DID NOT SEE THE MARK IN THE CHILD'S CHIN IN THE MORNING BEFORE THE CHILD WENT TO SCHOOL IN THE MORNING. HE ADDED THAT THE CHILD USUALLY GIVES HIM A HUG BEFORE GOING TO SCHOOL AND IF THE CHILD HAD A MARK HE WOULD HAVE SEEN IT. HE ADDED THAT THE CHILD MAY HAVE A SKIN PROBLEM TOO BECAUSE THE CHILD IS TAKING TWO MEDICATION THAT THE DOCTOR STD TO WHICH THE CHILD MAY DEVELOP SOM.E TYPE OF REACTION. HE ADDED HE WAS GOING TO CONTACT THE DOCTOR AND LET HIM KNOW. PI LET HIM KNOW THAT HE WOULD CONTACT CPT TO KNOW IF CPT WANTS TO SEE THE CHILD AND PI GAVE HIM THE PHONE # OF CPT AND ASKED HIM TO CALL CPI TO FIND OUT ABOUT THE APPOINTMENT. HE ADDED THAT HE DID NOT WANT ANY PROBLEM BECAUSE HE IS ADOPTING THE CHILD AND HE DOES NOT WANT ANYTHING TO CREATE A PROBLEM FOR HIM. I-[E STD HE HAD FIVE CHILDREN IN THE HOME AND ONLY ONE IS HIS BIOLOGICAL CHILD. ALL THE OTHER ONES ARE FOSTER CHILDREN. PI EXPLAINED TO HIM THAT THE CHILD ALSO HAD A SCALP PROBLEM TO TAKE CARE OF IT.

SHE STD SHE WAS IN THE CLASS AND WHILE TI-IE STUDENTS WERE DOING AN EXERCISE SHE NOTICED A RED BRUISE BY THE CHILD'S CHIN, SHE TOUCHED IT AND THE CHILD STAD IT HURTS AND THE CHILD TOLD HER THAT SHE FELL AT HOME. SHE ADDED THAT THE CHILD TURNED AROUND AND TOLD THE COUNSELOR THAT SHE FELL AT HOME. SHE STD SHE DID NOT KNOW WHAT TO BELEIVE. SHE ADDED SI-IE ASKED THE CHILD TO SHOW HER WHERE IT HAPPENED THE CHILD COULD NOT SHOW HER AND STARTED TO PULLED BACK. SHE STD THE CHILD SOMETIMES COMES TO SCHOOL NOT 7 TOO CLEAN. SHE ADDED THAT THE CHILD STARTED THE SCHOOL VERY BEHIND AND SLOW FOR THE CLASS UT SHE STARTED TO CATCH UP. SHE ADDED THAT THE CHILD STARTED SCHOOL ON NOV 17 AND THE FOSTER PARENT TOLD HER THAT SHE SHE DID NOT HAVE THE PROPER DOCUMENT TO REGISTER THE CHILD AT SCHOOL THAT IS WHY IT TOOK HER SO LONG.

PI GOT THE ATTENDANCE AND THE GRADE FOR THE CHILDREN. NUBIA HAS 17 ABSENCES FROM NOVEMBER TO FEBRUARY THE 23RD. SI-IE DOES NOT HAVE REGULAR GRADES AS SHE IS IN KINDERGARTEN.

PI WENT TO THE HOME AND FOUND THE CLEAN AND NEAT. PI ALSO FOUND THE FOSTER PARENTS TAKING NUBIA A SHOWER AND WASHING HER HAIR. PI INTERVIEWED THE FOSTER PARENTS WHO REFUTED THE ALLEGATION. PI INTERVIEWED THE TEACHER WHO WAS [NCONCLUSIVE ABOUT THE ALLEGATION. PI INTERVIEWED COUNSELOR WHO WAS INCONCLUSIVE ABOUT THE ALLEGATION. PI REFERRED TO TO CPT FOR EVALUATION AND THE ASSESSMENT OF CPT WAS NO CHILD ABUSE WAS FOUND. PI REFERRED THE CHILD TO CPC FOR EVALUATION FOR HIPPER ACTIVITY AS REQUESTED BY CPT AND THEN CLOSED THE CASE WITH NO INDICATOR.

FOR THE PAST FIVE MONTHS, NUBIA HAS BEEN SMELLING AND APPEARING UNKEMPT AT LEAST 2 OR 3 TIMES A WEEK, NUBIA SMELLS. SHE SMELLS ROTTEN. HER UNIFORM IS NOT CLEAN AND HER SHOES ARE DIRTY. ON ONE OCCASION, NUBIA GOT APPLE SAUCE IN HER HAIR, THE NEXT DAY SHE HAD APPLE SAUCE STILL IN HER HAIR. XXXX ALSO APPEARS UNKEMPT. ON 2/20/07, NUBIA HAD FOOD IN HER BACKPACK FROM BREAKFAST AND LUNCH. THERE IS CONCERN THAT MAYBE SHE IS NOT EATING AT HOME. NUBIA IS ALWAYS HUNGRY AND SHE EATS A LOT AT SCHOOL. NUBIA IS AFRAID TO TALK. 24 HOURS

SHE INFORMED THAT SHE EATS BREAKFAST EVERYDAY IN SCHOOL. STD THAT SHE EATS DINNER AT HOME EVERYDAY. STD SHE EATS THINGS LIKE CHICKEN, RICE, MACORONI, HOT DOGS, SPAGETTI, RAVIOLI. SHE INFORMED THAT SHE BATHES EVERYDAY AND MO SOMETIMES WASHES HER HAIR. STD THAT HER MO WASHES HER SCHOOL UNITFOR,. SHE GOES TO SCHOOL IN A CLEAN UNIFORM EVERYDAY. PI MADE A HOME VISIT AND F/F CONTACT WITH THE FOSTER PARENTS AND ADDRESSED THE ALLEGATIONS. THE FOSTER MO DENIED THE ALLEGATIONS. SHE INFORMED THAT SIE WASIES THEIR UNIFORMS AND THEY GO TO SCHOOL CLEAN EVERDAY. SHE SHOWED PI TIE LAUNDRY ROOM AND SHOWED PI TIE CLEAN CLOTHES ON THE HANGERS. PI OBSERVED 6 UNIFORM SHIRTS FOR THE TWO FOSTER CHN. THEY EACH HAVE A BLUE, A WHITE, AND A YELLOW SHIRT. PI DID OBSERVE TWO YELLOW SHIRTS TO APPEAR DINGY AND DISCOLORATED. PI ADVISED TIE FOSTER MO TO THROW THOSE SHIRTS OUT AND BUY THE CHN NEW ONES SHE NEEDS TO. TIE FOSTER MO TOOK PI TO THE FOSTER CHN'S ROOM AND SHOWED PI TIEIR SHOES. PI OBSERVED WHAT SHE CALLS THEIR SCHOOL SHOES', BUT ALSO SHOWED PI ABOUT 5 OTIER PAIRS OF SHOES. SIE INFORMED THAT TIE CHN GO TO THE AFTER SCHOOL PROGRAM AND SHE PICKS TIEM UP THERE AT ABOUT 3PM. THEY GO HOME AND TIE CHN PLAY OR USE TIE COMPT, OR WATCH TV. SHE INFORMED THAT SHE HELPS THE CHN WITH THEIR SCHOOL WORK. STD THAT CHN WERE DOING BETTER IN TIE BEGINING OF TIE SCHOOL YESR, BUT AFTER THEY STARTED GOING TO COURT AND TAKING TO A PSYCHIATRIST ABOUT THE ABUSE, THEY HAVE BEEN ACTING OUT. STD THAT XXXX IS SLOW, AND HAS PROBLEMS WITH THE WORK. SHE INFORMED THAT THE CHN BATH DAILY, AND WHEN THEY GET DRESSED IN THE MORNING THE CLOTHES IS ALWAYS LAID OUT FOR THEM. STD SHE COMBS THEIR HAIR EVERYDAY. STD THAT SHE EVEN HAS PERFUME AND COLOGNE FOR THEM. SHE SHOWED PI THEIR COLON GE AND PERFUME. SHE INFORMED THAT THEY USUSALLY HAVE DINNER BETWEEN 5~5:30PM. STD THAT NUBIA EVEN HAS DEODARENT, B/C SOMETIMES SHE HAS A STRING ODOR. THE FOSTER MO INFORMED THAT NUBIA IS ON MEDICATION XXXXXXXXXXXXXXX AS SHE IS A XXXXXXXXXX TAKING XXXXXXXXXXXX.

PI WENT TO XXXXXXXXX ELEM AND MADE F/F CONTACT WITH THE PRINCIPAL AND ADDRESSED TI-IE ALLEGATIONS. SHE INFORMED PI THAT THE CHN NUBIA AND XXXX ARE TWINS IN FIDDERENT CLASS. STD THAT THE TEACHER FORNUBIA HAS CONCERNS ABOUT HER IN REGARDS TO THE V/C GOING TO SCHOOL DIRTY AND OF BAD ODAR AT LEAST 2-3 TIMES A WEEK. STD THAT IN THE BEGINNING OF SCHOOL YEAR, NUBIA WAS ALWAYS FALLING ASLEEP IN CLASS.STD TILAT ONE TIME SHE FELL 3 ASLEEP IN CLASS WHEN TEACHER SAID SHE WAS TAKING HER TO THE OFFICE, NUBIA BECAME HYSTERICAL CRYING- SCARED TO GO HOME THEY CALMED HER DOWN BY TELLINH HER THAT SHE WOULD BE ABLE TO REMAIN IN SCHOOL, BUT THE MO TOOK HER HOME ANYWAY. SHE INFORMED THAT THE FOSTER MO HAS A SON IN THE SAME SCHOOL AND HE GOES TO THERAPY ON WEDNESDAYS. STD THAT SHE PICKS HIM UP EARLY AND ALSO TAKES THE OTHER CHN OUT OF CLASS EARLY. STD A THAT NUBIA IS MISSING LANGUAGE ARTS ON WEDNESDAYS, AND NUBIA IS GETTING RETAINED.STD THAT YESTERDAY THE MO SHOWED UP UNEXPECTEDLY TO PICK CHN UP. STD THAT NUMBIA STARTED TAKING OUR FOOD FROM HER BOOK BAG WHEN TOLD THAT FOSTER MO WAS THERE. STD SHE TOOK OUT A TOAST WRAPPED IN A NAPKIN AND TAPED WITH SCOTCH TAPE, A MIKL, A PACK OF CARROTS WITH DIPPING SAUCE, AND AN APPLE. STD THAT SHE RAN OFF THE CLASSROOM. SHE INFORMED THAT NUBIA IS ALWAYS HUNGRY. ACCORDING TO PRINCIPAL THIS WEK SOMETIME XXXXXXXXXXx GOT APPLE SAUCE ON NUBIA‘S HAIR. STD THAT NUBIA RETURNED TO SCHOOL THE NEXT DAY STILL WITH THE APPLE SAUCE IN HER HAIR. SHE INFORMED THAT THEIR UNIFORMS LOOK RUN DOWN ON THE DIRTY SIDE STD THAT XXXXXXXXX ALSO GOES TO SCHOOL DURTY AND WITH BIG UNIFORMS. SHE INFORMED THAT LATELY THE FOSTER MO HAS ASKED THE TEACHER TO PUNISH NUBIA B/C SHE IS NOT BEHAVING AT HOME. PRINCIPAL INFORMED THAT THERE IS NO FOLLOW UPS FROM THE FOSTER MO IN REGARDS TO HELPING TI-IE CHN.STD THAT SHE KEEPS NUBIA OUT OF SCHOOL ON FRIDAYS WHEN SHE HAS HER SPELLING TESTS, AND SHE TAKES THEM OUT OF SCHOOL EARLY ON WEDNESDAYS. ACCORDING TO PRINCIPAL, NUBIA APPEARS TO BE THREATENED OR SCARED OF THE FOSTER MOTHER. PRINCIPAL REPORTED THAT SHE SPOKE TO THE GAL ATTORNEY AND INFORMED OF WHAT IS GOING ON. SHE ALSO INFORMED THAT SHE HAS NEVER MET THE CASE MANAGER, AS HE HAS NEVER MET HIM.

Recently, Nubia has been acting out. Nubia`s hunger has been uncontrollable, she sneaks and steals food, steals money, has hair loss, is very thin, unfocused, nervous and jittery. Nubia has also had an unpleasant odor. Recently, Nubia also missed two weeks of school due to heavy bleeding. Nubia’s adoptive mother has said that Nubia's behavior can be attributed to hormonal issues. However, the mother has only provided personal documentation written from her place of employment. There has been no medical documentation from Nubia's doctor regarding her medical issues. It is unkr1own when the last time was that nubia saw her endocronologist. In the past, it is believed Nubia's adoption was halted when Nubia was coming to school dirty while in the adoptive mother's care.
A couple of months ago, Nubia had a scratch on her nose. It was said that Nubia rubs her nose a lot and that she is always falling,
